What is the best way to prepare for this one? Some people get 99th percentile. How do they do it, I am ashamed to ask.

Are you a surgeon? If you are in medicine, I would recommend MKSAP book series (Medical knowledge Self-Assessment Program, 12 books). The latest edition is coming out. The first 6 books are available, the other 6 will come in May. Usually the program buys these books for the third-years to ensure they pass the board. Ask any third-year to help you with the books. You also can buy them but better become a member of ASIM-ASP to get a discount. These books are published by the same organization that holds both in-service and board exams. Be aware that the books are volumous and take a lot of time to get through.

I am starting my surgical prelim (going into anesthesia after this). I don't really care much about surgery and the results of my surgical inservice exam.
How bad could it be if I got very low score on it?

Inservice exam is not about the specialty. It is about your desire to learn. I would say, scoring low will not surprise or dissappoint anyone, scoring high will get you some points.

How can is be "not about the specialty" if it has surgical questions (unlike anesthesia programs have anesthesiological ones)? Or inservice exams are the same for any residencies?

Yes, the questions are surgical, but they are more about basic science, less clinical.

High scores will impress anyone, even more so, if you are not from the same specialty.

Asked my program about it today. They said I don't even have to take this exam because I am not going into surgery. So, there was no reason to worry, silly me. :roll:

У меня 99% 3 года подряд :oops:
Резидентура ценит скор, т.к. ей самои достается если они ниже 50% percentile.
Я читала екзамены прошлих лет и MKSAP.
